Right after Dracula's carriage crashes he can be seen in the background running towards his coffin but in the next shot he is sitting on the ground.
When Talbot leaves the ice cave to search for Dr. Frankenstein's records, he has neither socks nor shoes on. But when he enters the next room, where the records are hidden, he's suddenly wearing shoes.
When Dr. Nieman helps Daniel out of the rubble, he lets go of Daniel's arm twice.
As the Wolfman thaws, his right hand is on his stomach and his left hand is on his chest. A single dissolve, and he is back to being Lawrence Talbot, but his hands have switched position - right hand on chest, left hand on stomach.
As Dracula's coach is being pursued by the police on horseback, Daniel pushes Dracula's coffin onto the road. Dracula sees what happened, but then the coach breaks loose from the hitch and hurtles down a ravine. Dracula races towards the coffin, which is now situated on the crest of a hill nowhere in proximity to where it landed.
When Larry Talbot emerges from the ice, his clothes are perfectly dry and pressed.
When Larry Talbot transforms into the Wolf Man for the final time, his hands aren't made up. This can be spotted before he crashes through the glass door.
When Dracula is thrown from the carriage, and looks up the hill to where his casket lays, half his mustache is missing.
As Lampini's wagon approaches the gypsy camp, the background behind Daniel still moves even after we see that the carriage has stopped.
When leaving the camp when they had 107km to reach Vasaria they close the wagon door where you still see rocks out the window of the door. The wagon starts shaking like it's moving, but the scenery doesn't change.
Just after the scene where Dracula dissolves in the sunlight, we see the wagons driven by Doctor Niemann and Daniel pass a sign reading "Frankenstein-1km". If you look past the sign, in the distance you can see a pile of film equipment cases and crates, and even bits of scaffolding used for lighting.
Just before 64 minutes, when Ilonka is alone on screen, you can briefly glimpse the shadow of camera equipment on the extreme right of the screen.
When Dr. Nieman and Daniel leave the prison ruins, Daniel picks up a lantern from the rubble. The electrical cable can be clearly seen as he moves through the tunnel towards the forest.
While searching for Dr. Frankenstein's records beneath the ruins of his castle, Dr. Niemann and Daniel discover the Wolfman and the Monster frozen. After thawing out the Wolfman, he then changes back to Lawrence Talbot. Dr. Niemann tells him he can cure him if he obtains the records of Dr. Frankenstein, last observed being read by Dr. Mannering in Frankenstein Meets the Wolf Man (1943) during a laboratory scene prior to the castle being destroyed. Talbot immediately proceeds to a another room where he reaches into the wall and miraculously pulls out the record book totally intact and undamaged.
In some earlier releases, when Daniel is trying to show Ilonka what a pentagram looks like, he draws a simple hexagon, not a five-pointed star.
What's even worse, they must have used multiple takes, because after a cut, the symbol gets even simpler, and now there is a pentagon on the paper where the hexagon used to be.
Later releases have edited this issue out.
When Ilkona prepares the pistol to kill Larry, she loads the bullet before the wadding. This would result in the bullet either not firing or not firing with enough pressure to be of any use.
