Director Michael Day chose a small-town playhouse to shoot the hospital scenes with lead actor Jake Abel, and supporting actors Greg Nutcher and Thomas Weissmuller. Weissmuller began acting in the same playhouse in the early 1970s. Fog of War was Weissmuller's first movie, prompting a pause and collective, "that's so cool" moment during framing as the shoot began.
