Any reason that https://github.com/alliedmodders/sourcemod/blob/master/.github/ISSUE_TEMPLATE/bug_report.yml couldn't be largely repurposed?

Nope, other than that template also needs some cleanup to flow a bit better. And it also needs to be updated to better clarify whether the user is using a dedicated server or a listen server. The latter have been rising in popularity with some recent game updates adding SDR support for them, but we still don't officially support it.
