Happens when Settings > Options > Appearance Advanced > Native Menus is disabled and you right click on an open tab.
Happens on a fresh vault. Only difference between the 2 context menus is that one has the "Change Icon" and "Remove Icon" option. Disabling the plugin removes the extra context menu. Doesn't happen anywhere else (Sidebar, links, etc.)

Thanks for the report! I'm not seeing this bug when I open a brand new sandbox vault, so I'm mystified as to what's happening there. Which OS do you use? No other plugins running?
Thanks for the plugin and looking into it! Resolved now! I had to redownload Obsidian and install it. Updating from the settings panel didn't do anything.
For context, OS was MacOS. Your comment prompted me to check on Windows and it wasn't happening there. I checked on my Mac and it looks like right clicks emitted 2 context menus on top of each other whether any plugins were installed or not. It was just noticeable with Iconic because it only added an entry to one of the context menus.
Anyway, getting a fresh installer solved it. Thank you!
