Add basic support for fanout queues & minor additions

New features:
  * Add support for fanout queues. This is basic but working.
  * Allow groups to be specified using a group directory - useful for when an
    agent namespace is the same as the group name
  * The broker URI can be specified as a environment variable.
  * smithctl can take an option to specify remote agency host. This allows all
    agencies to be controlled from one machine.

Fixes:
  * The agency sanitises the pid file.
  * Fix group bug in the list command.
  * The threadpool size is not set to 1. This can be set to anything now.
